This is not a timeline.  Every situation is different, and there are no fixed time frames for adjusting. This is just a framework to give families an idea of what to expect , and how they may be able to support their loved one during this transition time.

Week 1: Settling In

☐ Bring familiar personal items to help anchor the new space
☐ Share important background information with staff (routines, preferences, triggers, comforts)
☐ Keep explanations about the move simple and reassuring
☐ Expect increased confusion or emotional reactions during this period
☐ Coordinate visit timing with staff to avoid overwhelming transitions.

Weeks 2–3: Building Routine and Trust

☐ Observe how your loved one responds to the daily routine
☐ Ask staff how adjustment is going from their perspective
☐ Note changes in mood, sleep, or appetite without assuming permanence
☐ Allow staff time to establish rapport and consistency
☐ Adjust visit length or frequency if visits increase distress

Weeks 3–4: Communication and Fine-Tuning

☐ Review the initial care plan and discuss any needed adjustments
☐ Share what you are noticing at visits and ask for staff feedback
☐ Clarify how the team prefers to communicate updates or concerns
☐ Ask about activity participation and engagement
☐ Begin shifting focus from logistics to connection during visits

Ongoing Throughout the First Month

☐ Focus on patterns rather than isolated difficult days
☐ Give the environment time to feel familiar
☐ Be patient with emotional ups and downs
☐ Take care of your own emotional needs as well
☐ Reach out for guidance rather than waiting for a crisis

A Note

Adjustment to memory care is rarely immediate. Progress often comes in small, uneven steps. Early challenges do not mean the placement was a mistake, they are part of the transition process.
